  1. the number of faculty employees at the University of Northern Iowa (here, and throughout this e-mail, "faculty employees" is meant to include visiting professors, assistant professors, associate professors, professors, adjuncts, and other non-student employees with teaching responsibilities)
  2. the number of faculty employees who, if their own accumulated sick days are exhausted and have not met the elimination period for long-term disability, qualify to use the sick leave bank (here, and throughout this e-mail, the "sick leave bank" referred to is the one described here)
  3. the number of faculty employees who have used any sick leave bank time in each of the past three years
  4. the number of currently employed faculty who have ever previously used, or are currently using, sick leave bank time
  5. the number of total days used from the sick leave bank in each of the past three calendar years preceding the current calendar year
  6. for each faculty employee who used any time from the sick leave bank over the past three calendar years preceding the current calendar year, the total number of days of such time drawn in each calendar year during which they used such time
  7. the total number of faculty employees who covered courses scheduled to be taught by any faculty employees using sick leave bank time over the past three calendar years preceding the current one
  8. the total compensation paid to each faculty member specifically for coverage of courses scheduled to be taught by any faculty employees using sick leave bank time (that is, their compensation for such course coverage, ignoring their ordinary salary and benefits compensation) over the past three calendar years preceding the current one
  9. the number of human resources employees involved in the administration of the sick leave bank, and the estimated total hours spent weekly on administering the sick leave bank
  10. a list of all reasons for a faculty employee's use of sick leave bank time, with the number of faculty employees having that reason as a basis for their use of sick leave bank time, over the past three calendar years preceding the current one
